Home-at-Last
What?
When they are about to go home after a hospital stay, patients may be
eligible to receive assistance from a Personal Support Worker (PSW).
Clients who receive support during the first hours after coming home
from hospital are less likely to be readmitted unnecessarily.
How?
The PSW meets the patient at bedside, accompanies him or her home and
helps the patient settle in at home. It’s possible to make stops along
the way to pick up medications, equipment or necessities. The client
can also take home complimentary Meals on Wheels dishes so the fridge
won’t be empty.
Fee?
The Home at Last program is funded by the Ontario government. Eligible
clients don’t have to pay for the services, including follow-up visits
after the client gets home. The program doesn’t fund medications or
groceries.
Next
steps? Home at Last services are provided through
a partnership between seven of Toronto’s hospitals, nine community
service agencies such as SPRINT and CCACs (Community Care Access
Centre). Patients about to be discharged will be told about the service
by their hospital discharge coordinator. Of course, patients can also
inquire about Home at Last. In the SPRINT catchment area, the program
is now available at Mount Sinai, St. Joseph’s Health Centre, St.
Michael’s Hospital, Sunnybrook and Toronto East, Toronto General and
Toronto Western hospitals.

Home care services for seniors
All of SPRINT home care services are
provided by college-trained Personal Support Workers or other specially
trained staff.
Homemaking
– home care. SPRINT can provide a range of home
care to help you and your family member live at home independently and
safely. Some seniors need help with homemaking tasks such as light
cleaning, laundry, grocery shopping or an escort to a medical
appointment.
Personal
care – home care. Some elderly seniors can only
live at home safely when they receive help with personal care such as a
bath or medication reminders.
In-home
respite care (day/evening/overnight) – home care.
Many families search for respite care. It can be exhausting to provide
daily care for a family member with physical and/or cognitive
impairments. SPRINT staff can give such caregivers a break and engage
the care recipient in appropriate activities. This service is available
daily, including overnight. It gives caregivers peace of mind and
opportunity to maintain their own health and well-being. Please, also
check out information on our Adult Day Programs.
How?
Call our Information Desk at (416) 481-6411 to discuss your needs for
home care.
Fees?
Many seniors and their families arrange to pay privately for home care
services provided by SPRINT Personal Support Workers. Since SPRINT is a
non-profit agency, our rates are competitive with other providers in
North Toronto. Moreover, once you start working with SPRINT, you have
easy access to other practical services such as assisted
transportation, Meals on Wheels, and so on. Please review our fee
schedule. Some clients who require personal home care services may be
eligible for their services to be covered by Community Care Access
Centres, an agency with the province of Ontario. CCACs provide an
access point to health and personal support services to help
individuals live independently at home or assist them in making the
transition to a long-term care home. You can learn more at www.ccac-ont.ca
Next
steps? Contact the SPRINT Information Desk at
(416) 481-6411 to arrange home care services. You can also email InformationDesk@SPRINT-homecare.ca.
